The PSA Hardcore Championship is the epitome of uncontrollable violence. The Title was originally retired in Season 2, but its grand return in Season 11 brought a new wave of bloodlust across the entire PSA roster. During its entire lineage, the Title has been held by 2 people across 3 different reigns. Plenty of wrestlers claim to be extreme, but the only way to prove it is to become PSA Hardcore Champion.

"The Shepherd of Fire" proved himself right when he bested Andrew Frankowski in a Hardcore Classic match at Plugged In 2025 to become the two-time Hardcore Champion!

Drew Pender become the first-ever PSA Hardcore Champion after he was gifted the title by the General Manager, Cory Parcher. His historic 169-day reign came to an end when him and Corwin unified their respective Championships in a Last Man Standing match at PSA: Issue 22.
